As expected, ailing Los Angeles-based Maxicare Health Plans Inc. reported a $59.5-million loss for the second quarter. That compares to a loss of $8 million in the year-ago period. Maxicare, the largest publicly traded health maintenance organization in the nation, cited the cost of servicing debt and increases in medical costs.
